Our application packet contains numerous pages of information, along with the application itself.   We have also included a list of other senior communities and pertinent information along with a checklist to make sure you are returning all pages of the application, please refer to it before you mail/deliver the application to us.

If you have any questions about the application process or how to fill out the application, please call us at 503-240-4198.  Our office hours are Monday 11:00am to Noon and 1:00pm to 4:00pm, Tuesday & Friday 10:00am to Noon and 1:00pm to 4:00pm, and Thursday 10:00am to Noon. (Closed on Wednesday).

If you are going to mail the application, our address is: 6611 NE Martin Luther King Jr. Blvd, Portland, OR  97211-3073.  If you plan on dropping off your application its best to arrive approximately 30 minutes before the office window closes so we can check the application for accuracy and make photo copies of your current valid picture id and social security card; both are part of the application requirements. If you would like a tour, please call ahead to make an appointment as tours are by appointment only

IMPORTANT NOTE:  It may be that you prefer to only print specific pages, but it is very important that you print the complete application and fill it out before you mail it to us or drop it off at the office.   A completed application is required before a name is added to our wait list.
